We have an infusion team consisting of 4 RN's for house-wide care of all central lines including permacaths. We have developed policies based on best practice and CDC guidelines for care of the line while in place. One of our policies states if the dressing is compromised at all it must be changed "immediately". This puts the onus on the RN caring for the patient to be accountable for that line.

DOES ANYONE HAVE INFORMATION ON FLUSHING PROTOCOLS FOR CENTRAL LINES? OUR FACILITY STARTED USING CLC 2000'S LAST YEAR (POSITIVE PRESSURE DEVICE). WE STOPPED USING HEPARIN AND ARE FLUSHING WITH SALINE, BUT IT APPEARS WE HAVE MORE CLOTTING ISSUES.I WAS WONDERING WHAT THE PRACTICE WAS FOR OTHER FACILITIES . HOW OFTEN AND HOW MUCH SALINE OR IS HEPARIN STILL BEING USED? THANKS

Is there research that states how long before the central line insertion procedure that it is acceptable to clip the site?

Looking for any evidence to prove or disprove the value of a core group of staff (IV or infusion team)being responsible for implantable port access vs. an entire staff (about 60 nurses)being responsible. In our unit, this is a low volume activity occuring about once a day. Seems logical but can't find any hard evidence.

Hi Mari.
i have just read your message in IHI dated 08-15-2006 and i am surprised with your results.
We have used clc2000 for almost two years with very few accidents and we only do flush device when we stops the perfusions! After some positive experiences, and because the price of the Braun devices are less expensive we are using it for near one year also with very few accidents, and we only do the flush of the device when we stop the medications and just before starting a new medication at the lumen concerned.

We have developed a nursing checklist that we include with the kit - it also captures the daily assessment of the line necessity. Not sure how to attach it to this email but please contact me and I would be happy to share. I am looking for some physician national benchmarks related to maxium barrier and or all of the CV bundles. Anyone know where I can find? Krichey

Karen,
I would be interested in your line necessity checklist. We are a new hospital and I have recently been asked to participate in the CVL bundle. We have implemented the bundle but are trying to gather the data in a more efficient manner. Do you do it physically or do you pull your data from a computerized charting?
