It is not surprising that River's active treatment group received more RBC transfusions in the first six hours than did the control group. That is what the treatment protocol required. What is unclear is whether the transfused patients did better than the non-transfused patients. The TRICC trial patients did worse with transfusion when maintained at an HCT >30. Given the multitude of interventions is River's paper has anyone tried to tease out the benefit or lack of benefit of early transfusion?
