The example was very helpful to see, however I did not find it user-friendly in terms of being able to use the format itself in my office. The table was created using MS Visio, which I do not use nor does my IT department have loaded on our network. So I am unable to make any changes in the example provided, in order to arrive at a "blank" to copy and use.
I suggest that examples provided either be 1- created in more commonly used software such as MS Word, OR 2- when supplied in different formats than most commonly available (maybe smaller practices use Visio a lot; in large networked systems, choice of software is not always up to the end users), that IHI make it available in other formats such as MS Word.
This gets to the issue of who does the work, the "overhead" for tailoring materials to a given practice. I do not have an answer. Of course it is always nice when tools for use in practice can be downloaded and used "as is". The visual layout and example is helpful to those of us who are visual processors - I can get a better idea of how this might work in my office seeing the table than I could by reading a description of it - so there is value in having it posted in any form. I leave it to the discretion of IHI as to who it is that needs to do the work of making the table usable in practice; at this point, for this tool, looks like that is my shop which needs to adapt it to our needs.
