I am working on my thesis related to converting an obstetrical unit to a culture of safety. I completed the safety survey on my unit and I have a question- in developing a "high reliability organization" it is encouraged to always remain on alert for accidents and to never feel "safe" so that it takes away your thinking about how to deal with an adverse event. HRO's challenge the environment constantly so if you had a high postivie safety score does this imply a lack of knowledge or ability to respond to accidents since the staff feels safe? Or does it imply a recognition of the need for safety in everything they do and a culture that is safe based on the ability to respond to accidents?

Safety is an issue on the mental health unit and could be very costly. I am currently working with the CNM on reducing the number of 1:1 supervision by 50 percent in 6 months.
