We are planning to use the Safety Climate survey tool hospital wide at our hospital. May we have permission to use this exact survey at our facility?

We are an NHS Trust in the UK and are planning to use the safety climate tool . We would be interested in hearing how other sites are using it , especially the expected response rate . We aim to survey every six months from sept following a series of tests .Is anyone able to help us compare results ?

The survey material and the Excel spreadsheet will undoubtedly be widely used in healthcare. I'm sure that I am not the first one to notice that they could easily be tailored to other high hazard industries.

I have notified some colleagues of the existence of this material and will keep you advised of non-healthcare uses I become aware of.

I would also like to know the reliability and validiy statistics of this tool. Also, the rationale for question #18 being stated in the "reverse" of the other questions??

Thank you for providing this tool. I am currently working on my graduate thesis entitled "Establishing A High Reliability Obstetrical Unit by Establishing a Culture of Safety". This tool will assist me in this work and I intend to use it to survey multiple disciplines including nurses, unit staff, physicians- all OB care providers as well as anesthesiologists.
