Quality Improvement in Community Hospice

I am the Quality Coordinator in a 20 bed Community Hospice which provides short term palliative care for patients with a progressive illness for whom cure is no longer possible and require - palliation, - symptom control, - comfort and care during the last stages of life.
I would like to hear from anyone working in a similar setting with a view to sharing ideas.

I am a Quality Improvement Manager at Center for Palliative Care Studies. We work with various organizations and provide them consulting to improve end of life care. I would be happy to talk to you about our experience in the field. We have a book on end of life care which can be read online at http://www.medicaring.org/educate/navigate/sourcebook.html. This book is a result of two national collaboratives we did with IHI on end of life care. This book can provide you various insights in all the issues you mentioned. Most of our teams have worked in advance care planning and pain and symptom management. Some have been working on developing palliative care programs, increasing referals to hospice or palliative care consult programs etc.

Good to hear from you. There are lots of us working on improvement activities in such settings. You'll find an entire book on the web that provides some guidance - Improving Care for the End of Life - click on the cover picture at www.medicaring.org We are signing interested people up for a collaborative venture in improving care to serve those with eventually fatal chronic conditions - you can see that at the same website. Let me know if you have particular challenges that you hope to address and I can probably help put you in touch with others.

Good Evening.... My name is Melissa and I am a beginning nursing student and I am very interested in the hospice nursing...the end of life I believe is just a transition....I would like to know if there are specific classes that would help me in this area of training...Again, I am new but very interested

I would suggest reading some literature such as Improving Care for the End of Life by Joanne Lynn, MD also Palliative Care Nursing by Matzo and Sherman. There is also a testbook of Palliative Nursing by Ferrell and Coyle. I would also suggest you volunteer at a local hospice.

Would you happento have any insight for the inpatient palliative care program? I have been working to start facilitating the creation of a Palliative Care consult team. I am looking for guidelines, forms, polcies/procedures, etc. I have reviewed the CAPC website and others. Just need the next step! Thanks

"5 wishes" is a very wonderful "pamphlet" to read and give to your patients. Also, I would concur that the end of life could be seen as a transition-but not JUST a transition. Good to read about different cultural beliefs. Good luck..you are so much needed in this rewarding field of nursing. Many hospitals have hospice beds or a separate hospice affiliation. Saratoga Springs,N.Y. has a hospital inpatient hospice.
