Adverse Drug Events (Medication Reconciliation)
Prevent Adverse Drug Events (Medication Reconciliation) is one of six interventions in the Platform of the 100,000 Lives Campaign. Medication Reconciliation is a formal process of obtaining a complete and accurate list of each patient’s current home medications – including name, dosage, frequency, and route – and comparing the physician’s admissions, transfer, and/or discharge orders to that list.
