This protocol is for use by Risk Managers and others trained in incident investigation. You will be able to use adverse events as opportunities for learning and to promote a climate of greater transparency. You will learn about a framework for carrying out an incident investigation and analysis and how it can apply to various, if not all, health care contexts.
