There have been massive gains in terms of improvement to patient services over the past five years. Evidence from England has demonstrated that around a third of improvement projects fail to hold the gain or continually improve beyond the formal end of the project and this is consistent with the available literature. We have developed a model consisting of eleven factors, which we consider to be important in terms of sustainability of improvement efforts. This focused discussion seeks to consider and debate these factors and their relevance to your improvement work. We would also like to consider and debate the actions that could be taken to address problem areas identified through use of the model.
