Making Progress on a Non-punitive Approach to Errors
Errors are made all of the time. They are made by smart, caring people working in complex systems. Most systems continue to punish people when they make mistakes. What can be done to change this punitive approach? Don't people need to be accountable for their actions? How can we strike a balance between a non-punitive environment and maintaining standards?
