Research shows that safety culture is relatively poor among front-​line workers in skilled nursing facilities (SNFs). Trigger tools can help identify opportunities for patient harm and measure adverse events that occur over time. In the IHI Virtual Expedition: SNF Trigger Tool – Measuring Patient Harm in Skilled Nursing Facilities, participants will learn how to efficiently use the IHI SNF Trigger Tool to identify patient harm. Further, teams engage in a collaborative learning community — consisting of peers and IHI staff — to learn how to use their results to measure the effectiveness of safety improvements to reduce patient harm. 

The IHI Skilled Nursing Facility Trigger Tool for Measuring Adverse Events provides an easy-to-use method for accurately identifying adverse events (harm) and measuring the rate of adverse event incidence over time in SNFs. Identifying adverse events in SNFs, and the types of harm resulting from such events, can lead to opportunities to improve patient and resident safety. ​​​​​​​​​​​​​​​​


​​At the end of the Expedition each participant will be able to:

  • Conduct patient record reviews using the IHI SNF Trigger Tool
  • Summarize how to use the data collected with the IHI SNF Trigger Tool
  • Describe the infrastructure needed to implement the IHI SNF Trigger Tool
  • Determine the presence and rate of harm in patient records
  • Identify the categories of harm detected with the SNF Trigger Tool
  • Be part of a learning community focused on improving safety in SNFs
