Federico, RPh, Vice President, Institute for Healthcare Improvement, works in improving patient safety, applying reliability principles in health care, preventing surgical complications, and improving perinatal care. He serves as faculty for the IHI Patient Safety Executive Development Program and has co-chaired a number of Patient Safety Collaboratives. Prior to joining IHI, Mr. Federico was Program Director of the Office Practice Evaluation Program and a loss prevention and patient safety specialist at the Risk Management Foundation of the Harvard-Affiliated Institutions. He has published numerous patient safety articles and co-authored a chapter in Achieving Safe and Reliable Healthcare: Strategies and Solutions, and he was an executive producer of the film First, Do No Harm, Part 2: Taking the Lead.
Carol Haraden Carol R. Haraden, PhD, Vice President, Institute for Healthcare Improvement, is a member of the IHI team responsible for developing innovative designs in patient care. She leads IHI's work with Health Improvement Scotland to transform the safety of every Scottish hospital over a five-year period. She also leads work in Denmark, the South of England, and the US to improve the safety of health care systems in these countries. Dr. Haraden is the Executive Lead for the IHI Patient Safety Executive Development Program. She has been a dean in higher education, as well as a clinician, consultant, and researcher, and she served on the Institute of Medicine Committee on Engineering Approaches to Improve Health Care. Dr. Haraden is a judge for several national quality awards and an associate editor for the journal BMJ Quality and Safety.

  • Introduction to the workshop case study 
  • Using a Mortality Diagnostic Tool Refreshment Break 
  • Using Root Cause Analysis to understand cause and effect
Afternoon Session (1:45 PM–5:00 PM)
  • Using Trigger Tools to identify systems issues
  • Refreshment Break
  • Using FMEA — a more proactive approach
  • Linking diagnostics to improvement — bringing it all together
