Monday, September 29
8:15 AM - 9:15 AM​ Welcome and Setting the Stage
​Trissa Torres
​This session will kick off with an introduction to the program agenda and IHI. Participants will also receive an overview of IHI’s framework for population management.
​9:15 AM - 10:30 AM Plenary: Leading in a New Era and Accelerating Your Journey
George Kerwin
​This session will review what it means to lead in a new era of payment and delivery reform. Bellin Health’s journey will be described. This session will also describe the five accelerators to improve population management (leadership, integrated data systems, improvement infrastructure, provider and stakeholder engagement, and financial models.)
10:30 AM - 10:45 AM Break
​10:45 AM - 12:00 PM World Café Exercise
​This exercise will give participants a chance to begin using IHI’s framework for population health improvement in small groups. Groups will rotate among five stations and will think through the collective assets and barriers of the organizations in the room related to the five accelerators noted above. This is an opportunity to get up and moving, start connecting, and discuss with one another.
​12:00 PM - 1:00 PM Lunch
​1:00 PM - 2:00 PM Plenary: Using Data for Population Management
​Gordon Moore
​This plenary will address common challenges and possible solutions related to integrated data support for population management. Topics will include: Investment in EMR and IT infrastructure, limitations of existing IT systems in terms of analytics, working with multiple EMRs, and providing timely and actionable data to clinicians to drive performance improvement.
​2:00 PM - 2:45 PM Case Study: Partners Healthcare
Clay Ackerly
​This session will involve an in-depth case study of one system’s efforts to identify, segment, and truly understand a select patient population. Participants will hear a firsthand account of how Partners Healthcare operationalized this within their own organization.
​2:45 PM - 3:00 PM Break
​3:00 PM - 3:45 PM Reflection and Action Point: Leadership and Using Data
​This timeframe will be an opportunity for participants to connect with appropriate members of their virtual team. During this time, teams can debrief and reflect on what they have just heard and begin to map out their organizational assets and barriers related to identifying, segmenting, and understanding the wants and needs of their designated patient population.
​3:45 PM - 4:30 PM Burning Questions Panel
​As part of the pre-work for this course, participants are asked to reflect on and interview others about the “burning questions” they have that they hope this program will address. This session will be an opportunity for the faculty to respond to some of these questions and engage with participants in a facilitated discussion.
​4:30 PM - 5:00 PM Wrap Up of Day One and Setting the Stage for Day Two

This plenary will cover ways in which organizations have developed successful partnerships with community organizations to address the needs of their collective population that fall outside the purview of direct care delivery.

​This will be an opportunity for the participants to connect with appropriate members of their virtual team, to debrief and reflect on what they have just heard, and to map out their organizational assets and barriers related to building effective community partnerships.

​2:45 PM - 3:30 PM Plenary: Leveraging Financial Models to Achieve Clinical and Financial Targets
