IHI is pleased to offer a limited amount of funding for scholarships and special discounts. Please note that scholarships and special discounts are available to assist with program enrollment costs ONLY and are not applicable for travel, food, or accommodation costs associated with attending the program. Applications must be submitted no later than 11:59 PM EST, Friday, November 30, 2018.
IHI is pleased to offer a limited number of 50% discounts for this program for employees of:
- Independent, United States Federally Qualified Health Centers (FQHCs) that are not affiliated with a hospital or health system
- Critical Access Hospitals
- Members of America's Essential Hospitals (formerly NAPH)
For the National Forum, IHI is pleased to offer these scholarships for general conferences fees ONLY. These scholarships are not available for IHI Quick Courses, Learning Labs, or Forum Excursions:
- 75% scholarship for full-time students
- 50% scholarship for part-time students (2 classes or more)
- 50% scholarship for residents
- 50% scholarship for full-time faculty and deans
For this program, IHI is pleased to offer a limited amount of scholarship funding to offset the seminar enrollment fees in cases of financial hardship.
Patient/Family Advisor Application
The Institute for Healthcare Improvement (IHI) recognizes the importance of working in partnership with patients and family members to improve the quality and safety of health care. At IHI’s National Forum, Patient/Family Advisors and Leaders contribute to the energy and transformative experience for all who attend.
This year, the IHI team is proud to provide funding to support the registration, up to $500 toward travel and three nights' lodging for a limited number of exceptional Patient/Family Advisors who are making a contribution to the field of person- and family-centered care. A Patient/Family Advisor is a person with lived experience as a patient or family member who is collaborating with a health care organization and shows success, drive, and interest in improving the patient experience.
Please be advised that each application will be judged solely on its merits and not on a prior application or relationship. Please answer each one of the application questions as completely as possible.
Accepted applicants will be asked to submit a brief blog about their Forum experience. Additional details about the blog length and due date will be made available at the time of acceptance.

The Justin A. Micalizzi Memorial IHI Scholarship
Justin’s HOPE Project and IHI are extremely proud to announce the Justin Micalizzi Memorial IHI Scholarship in loving memory of Justin A. Micalizzi who died at the age of 11 due to a medical error. Learning from this devastating loss ― and making a difference that will improve pediatric health care ― has become a quest for Justin’s family.
Justin's HOPE project will award scholarships to health caregivers who are committed to pediatric patient safety and providing a safe health care environment for their patients and families. The Justin Micalizzi Memorial IHI Scholarship covers registration fees for all four days of the National Forum on Quality Improvement in Health Care (a $2,450 value). The scholarship will also provide up to four nights complimentary hotel accommodation at one of the National Forum hotels (a $1,000 value), and reimbursement for travel expenditures up to $500.

Annette J. Bartley Memorial Scholarship
Annette J. Bartley RN, BA, MS, MPH, was a nurse and health care improvement coach for more than 30 years, which included several leadership roles in frontline clinical care and management. A beloved member of the Institute for Healthcare Improvement (IHI) community, Annette died in November 2016, leaving behind a legacy of kindness, compassion, and optimism.
IHI will award the Annette J. Bartley Memorial Scholarship annually to one nursing professional who demonstrates a strong interest in health care improvement. The scholarship will cover the cost of General Conference fees for IHI’s National Forum on Quality Improvement in Health Care. In addition, the scholarship will provide the winner with up to $1,000 for travel, four nights’ lodging, or other Forum expenditures (including fees for Learning Labs and Quick Courses).
