The National Forum carries a

How to receive a certificate of credit:
All attendees will receive an email with a link to take your surveys at the conclusion of the conference on Wednesday, December 13th. Attendees must complete the surveys within 30 days to obtain their continuing education certificate. To be eligible for a continuing education certificate, you must ensure you have selected your sessions prior to the end of each conference day.
