The biggest issue in ambulatory patient safety is the lack of a safety infrastructure. Primary care offices, surgery centers, dialysis centers, urgent care clinics, and outpatient pharmacies often lack the resources, tools, and mechanisms to adequately implement and manage safety initiatives.
