Jim Conway, Chief Operations Officer at the Dana-Farber Cancer Institute (DFCI) in Boston, developed this tool for public distribution within the health care community with the support of several organizations, including the Massachusetts Coalition for the Prevention of Medical Errors, the Massachusetts Hospital Association, and the American Hospital Association (AHA).

This tool reflects insights and lessons Mr. Conway and his colleagues at Dana-Farber Cancer Institute (DFCI) learned during their intensive years of focus on enhancing safety. It is meant to help guide hospital executives leading their own institutions in the journey toward a culture of safety, with input on areas such as the design of safe medication systems, interdisciplinary practice, and making patients and families true partners in care design and delivery.
