The Safety Climate Survey has been removed from IHI.org.

As of December 2006, the Safety Climate Survey (developed by Bryan Sexton, PhD, MA, and colleagues at The Center of Excellence for Patient Safety Research & Practice, University of Texas) is no longer freely available so it has been removed from IHI's website. 

IHI was asked to remove the tool and all related materials from our website. At this time, IHI does not have further information to share about where users may locate the survey elsewhere. If such information becomes available, we will post it here on IHI.org. 

Regretfully, IHI is no longer able to provide access to the survey or to answer questions from users who had previously accessed and downloaded the tool from IHI.org. Thank you for your understanding.
