Use the data collection spreadsheet to analyze and better understand a unit’s or hospital’s last 20 falls that resulted in injury. Continue to add information on injurious falls as they occur, to further track and communicate patterns, issues, and potential interventions.

The tool was created and tested as part of the Institute for Healthcare Improvement's "Reducing Harm from Falls," initiative that was largely funded by the Robert Wood Johnson Foundation.
