Iowa Health System has designed a number of initiatives to develop a culture of safety. Each of these has resulted in increased information about safety problems, errors, and adverse events. A database enabled the organization to track all of the appropriate information. And to ensure that the appropriate entities took action, the organization employed this database. Here a variety of information can be entered, updated, and tracked. You may find it a useful model as your organization develops its own database.

You must have Microsoft Access installed on your computer to use this file once it's downloaded. Windows XP will automatically open the ZIP file; for other operating systems a tool like WinZip (or other free alternatives) should be used.
