An unreconciled medication is one that has been omitted in the orders without explanation.  This includes medications listed as previously taken on a home medication list for admissions or listed on the MAR for transfers and discharges.  This tool may be used to track a patient’s medications upon admission, transfer, and discharge.

As a participant in the Institute for Healthcare Improvement’s Reducing High Hazard Adverse Drug Events Breakthrough Series Collaborative, the Baptist Memorial Hospital, Memphis campus has tested this tool on a pilot population to decrease their rate of unreconciled medications in order to improve patient safety.
