Institute for Safe Medication Practices
The Institute for Safe Medication Practices (ISMP) is a nonprofit agency that provides information about adverse drug events and their prevention to healthcare practitioners and institutions, consumers, regulatory agencies, professional organizations and the health care industry at large. In cooperation with the United States Pharmacopeia, ISMP operates the national Medication Errors Reporting Program (USP-ISMP MERP). Their website includes research findings, books and publications, an electronic discussion forum, and links to related sites.
