Based on error reports submitted to the Institute for Safe Medication Practices (ISMP) Medication Errors Reporting Program, reports of harmful errors in the literature, and input from practitioners and safety experts, ISMP created and periodically updates a list of potential high-alert medications.

The Institute for Safe Medication Practices (ISMP) is a nonprofit agency that provides information about adverse drug events and their prevention to healthcare practitioners and institutions, consumers, regulatory agencies, professional organizations and the health care industry at large. In cooperation with the United States Pharmacopeia, ISMP operates the national Medication Errors Reporting Program (USP-ISMP MERP). Massachusetts Coalition for the Prevention of Medical Errors

The Massachusetts Coalition for the Prevention of Medical Errors was established to improve patient safety and minimize medical errors. The goals of the coalition are to: Establish a mechanism to identify and implement best practices to minimize medical errors; Increase awareness of error prevention strategies through public and professional education; and Identify areas of mutual interest and minimize duplication of regulatory and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) requirements so that efforts are focused on initiatives that can best improve patient care.
