Improve Core Processes for Ordering Medications:
Collect Allergy Information on Admission

Lack of current allergy information can lead to the administration of a medication to which a patient is allergic. Be sure to collect allergy information at the moment of admission in all settings — inpatient, outpatient, and ambulatory surgery — so it can go on record before staff members order or administer any medications.


Tips
  • Record information immediately and make sure it is visible to anyone who may order or administer medications.
  • Make sure the information is transmitted to the pharmacy as soon as possible.
  • Place allergy information in multiple locations.
  • Ensure that the allergy information follows patients
  • Use information from previous admissions if the patient is unable to provide it upon this one.
