To facilitate the implementation of the Institute of Medicine’s recommendations in its landmark report, Crossing the Quality Chasm: A New Health System for the 21st Century, the American Hospital Association (AHA) partnered with the Institute for Family-Centered Care, to produce the Strategies for Leadership: Patient- and Family-Centered Care Toolkit. The toolkit contains the following:
• The Patient- and Family-Centered Care: Partnerships for Quality and Safety Video presents the core concepts of patient- and family-centered care, profiling a process of change in two institutions and featuring the perspectives of patients, families, clinicians, and administrative leaders.
• The Video Discussion Guide assists hospital leaders in facilitating discussions and developing an action plan for change.
• The Resource Guide provides information about the concepts of patient- and family-centered care, case studies, references, as well as guidance on how to advance such care within hospitals and health systems.
• The Hospital Self-Assessment Inventory helps evaluate existing patient- and family-centered care within a hospital and identifies opportunities for improvement.
