The Center for Medical Home Improvement is dedicated to establishing and supporting quality improvement teams that are composed of families and professionals to evaluate and enhance health care for children with special health care needs. CMHI offers a myriad of resources to develop and evaluate a “medical home,” including how to prepare and support families to be partners in these initiatives.
