Many resources to support people in managing chronic conditions are available outside the health care setting, in the communities where patients spend their time. A community-oriented approach to self-management support links people to the resources they need. This bibliography contains journal articles, books and chapters, presentations, websites, and other materials to facilitate the integration of an expanded range of community services into health care.
