Preventing Pressure Ulcers Turn Clock Tool

Owensboro Medical Health System
Owensboro, Kentucky, USA

The turn clock tool is posted to alert staff that this patient has been identified as being at risk for pressure ulcers. It serves as an important reminder to reposition the patient every two hours, an key component of care for at-risk patients.

Directions

Identify those patients at risk for developing pressure ulcers and post the Turn Clock on the doors to the rooms for those patients. The clock serves as a reminder to all who enter the room that at each two-hour interval, the patient should be re-positioned according to the position indicated on the clock. For example, from 12:00 to 2:00 the patient should be positioned on his or her back; from 2:00 to 4:00 on the right side; and so forth.
