This tool is used to document the presence of a wound and condition of surrounding skin on admission and at regular intervals thereafter to document wound progression. It is also used to document objective images of treatment outcomes for referral and payer services and to facilitate communication among interdisciplinary teams.
Directions
The downloadable tool contains two documents:
- Wound Photo Documentation Protocol
- Photographic Wound Documentation Form
Complete assessment of wound(s) on right side of the Documentation Form (location, classification, stage, size, color, drainage/odor, induration, margin erythema, and tunneling/underming. Include nurses signature, date and time. We place completed form, with patient ID sticker, in patient’s chart under physician progress notes.
**Please note that the downloadable tool file is in ZIP format, and contains two Word document files. Windows XP will automatically open the ZIP file; for other operating systems a tool like WinZip (or other free alternatives) should be used.
