Total number of discharged patients from the unit per month, divided by the number patients from the unit readmitted within 30 days to the same hospital/health system, multiplied by 100 to get the percentage.

Readmissions within 30 days are reduced to 5 percent or less.

Collect data on the unit monthly. Planned readmissions should be excluded (e.g., chemotherapy schedule).
