Medical-Surgical Care: General

Because content in this area is still being developed, it is posted in "document" form — and these documents will be updated as the innovation work progresses.

 

Promising new models of care for medical-surgical patients are emerging from collaborative projects such as Transforming Care at the Bedside (TCAB) and IHI's Taking Action to Transform Care at the Bedside Learning and Innovation Community

 

The Taking Action to Transform Care at the Bedside Community, which began in November 2005, focuses on the following areas:

  • Making care safe and effective: Examples include reducing medication errors and adverse drug events, establishing rapid response teams, and preventing inpatient falls.
  • Promoting care team effectiveness and vitality: Examples include optimizing communication among team members, engaging front-line staff, and incorporating talent management and professional development programs for staff.
  • Supporting and involving patients and families: Examples include support and involvement of patients and families and creating patient-centered healing environments.
  • Increasing the value of care and care processes: Examples include reducing waste through application of “lean” approaches, and improving work environments through space redesign.

 

The content in this section includes material that is emerging from both the TCAB project and the Taking Action to Transform Care at the Bedside Community.

Case for Improvement

A narrative description of the Transforming Care at the Bedside project, including background, problem statement, mission, goals, methods, and citations. Also included is a list of the thirteen sites who will be participating in Phase 2 of the project.


Measures

This document provides detailed definitions for each of the 21 measures being used by TCAB hospitals. Includes system, hospital, and unit level measures.


These measures help to assess the progress that hospitals participating in Transforming Care at the Bedside (TCAB) are making toward achieving the TCAB design targets for unit and system quality measures, as well as clinical measures for top diagnoses.

In the summer of 2003, the Institute for Healthcare Improvement and The Robert Wood Johnson Foundation gathered health care experts to explore methods of improving bedside care in medical-surgical units. The session, called a Deep Dive, was facilitated by IDEO, a design and innovation consultancy, and captured on film.
