This scale, adapted from the IHI Breakthrough Series Collaborative Assessment Scale, provides information on how to assess a team’s progress throughout a Collaborative-type improvement project.
Background
The Collaborative Assessment Scale was originally developed at IHI in order to assess teams participating in IHI Breakthrough Series Collaborative projects. This tool allows Collaborative Directors and Improvement Advisors to determine how well teams are doing, on a scale of 1 to 5, in meeting improvement goals and implementing changes. This tool is a Dutch adaptation of the scale, developed by Steven van Berlekom of the Rehabilitation Center De Hoogstraat.
Note that this scale differs slightly from the IHI scale because the aim is to monitor the development of the improvement itself, not just the work of the project team.
