This tool may be used by the case manager to facilitate discussion with patients/clients around their needs and to identify a plan to meet those needs.
The case manager should walk through the Wellness Plan: Client Needs Assessment sheet with the patient/client and identify the major areas in which the client needs help. For each need identified, fill out the Wellness Plan Goal/Need form. Outline three things the client will do to meet the goal and three things the case manager will do to help the client. Make sure to schedule the next appointment and mark it in the top right of the Wellness Plan/Goal sheet, and review the steps outlined to achieve the goal during the next visit.
