This tool is designed to be used by case managers with new HIV-positive clients to obtain the treatment history and health-related, social, and psychological information needed to develop effective care plans.
Directions
It may take several visits to complete the tool. Development of trust between the case manager and the client is critical for honest disclosure of information. Revisit the tool when information changes and/or needs to be updated.
