Self-management and adherence for people with HIV disease involves not just medication adherence, but all aspects of general health and life balance. In using this tool during a visit, the clinician and patient/client partner to review self-management goals, identify barriers to achieving optimal self-management, and create solutions to remove or lessen the impact of those barriers. Review the contract at each visit to note patient/client improvements and identify new goals.


Background

This tool was developed to promote communication between providers and patients/clients not only about medication adherence but also a broader range of self-management issues.


Directions
During a patient visit, walk through the one-page sheet to identify key challenge areas for the client/patient. Write notes on the back. Focus on one or two areas at a time, so as not to overwhelm the patient/client.
