Antiretroviral Medication Adherence Survey (English and Spanish Version)

The Antiretroviral Medication Adherence Survey serves as a quantitative assessment tool to measure medication adherence. The survey may also be used as a counseling tool based on the charts in the article, "Prevalence, Predictors, and Outcomes of Early Adherence after Starting or Changing Antiretroviral Therapy," portraying the virologic response at six months.


Directions

The scoring of the questionnaire is as follows. For items 1-5, the clinician writes in the names of all the patient’s antiretroviral medications, using either trade name or generic or both (whatever the patient will understand best). Then the patient is asked to fill in the number of missed doses for each medication for each of the four preceding days. For each medication, the sum of the number of missed doses across all four days is calculated and the two highest (or worst) scores are entered into the boxes labeled "Sum 1" and "Sum 2," respectively. For questions 2 through 4, the numerals in parentheses beside the response options indicate the number of points associated with each response. The Total Score is calculated as the sum of all item scores: Sum 1 + Sum 2 + Score 2 + Score 3 + Score 4.
