Percent of Patients/Clients with Self-Management Goal Setting


Definition

Participation by patients/clients in establishing goals for treatment and monitoring their own care will contribute to greater emotional well-being and will likely achieve better health outcomes. The numerator is the number of patients/clients with documented self-management goal setting within the last six months; the denominator is the total number of HIV/AIDS patients/clients in the registry. Divide the numerator by the denominator and multiply by 100.


Goal

Greater than 70 percent of patients/clients will have a documented self-management goal.


Data Collection Plan

On the last day of the month, count the number of patients/clients with documented self-management goal setting within the last six months; divide by the total number of HIV/AIDS patients/clients in the registry (multiply by 100).
