Improve Antiretroviral (ARV) Management for HIV-Infected Patients:
Utilize a Cross-Functional Team Approach
The benefits of a team approach to patient care are well researched and documented.  Given the complexities of treating HIV patients and incorporating the various challenges in engaging patients in care, a cross-functional care team might be best suited for addressing the various needs of patients in care.  Understanding and addressing the organizational barriers to having cross-functional team meetings can help HIV programs make them possible and efficient.
Tips
  • Form a cross-functional team for case conferencing that meets routinely to discuss certain patient cases (allow for addressing system issues).
  • Arrange meeting times conducive to provider schedules (e.g., lunch time) to increase staff participation from various backgrounds.
  • Make cross-functional meetings most effective by developing meeting agendas in advance, and agree on a facilitator.
  • Assess what went well and how to improve at the end of each meeting in order to improve the next meeting.
  • Use team meetings to discuss issues related to patient adherence, when to initiate and change ARV regimen, and how to assist the most vulnerable and sickest patients.
  • Develop a standard format for keeping minutes that clearly states decisions made about each patient’s circumstances — medical, socio-economic, mental, drug and/or alcohol use — outlines follow-up action, and who is responsible for completing action and when.
