Identify and Remedy Current Gaps in Community Resources
  1. Gather information on gaps by participating in planning bodies, performing needs analyses and focus groups, and conducting patient satisfaction surveys.
  2. Compare patient needs and resources. Identify existing community collaboratives.
  3. Perform a gap analysis, prioritize needs, and select resources.
  4. Maintain a resource inventory.
