Educate Patients about Delivery System Design

Create a patient orientation and self-management manual to teach patients about the new delivery system design.
Ensure that all new and existing patients are instructed about how clinic visits will be scheduled and conducted.
Teach patients how and when to notify the clinic of urgent care needs.

Tips

Display information regarding delivery system changes in a waiting room.
Ensure that a social worker is available to patients at every clinic visit in person or by cell phone/pager.
Post a bulletin board with self-management information and calendar of support group meetings in the waiting room.
Post staff photographs in the waiting room to acquaint patients with staff members.
Assess patient satisfaction scores and quality of care report cards. Keep a running tally of waiting time.
