Community:
Create and Maintain an Updated List of Community Resources and Ensure Distribution to Staff, Patients, and Families
  1. Identify existing lists of resources.
  2. Assign a staff member or volunteer to collate lists, update information, and review for accuracy. Collect contact information and guidelines for program use.
  3. Contact local, state, and federal resources:
    • Senior Information and Assistance Program
    • Local library
  4. Use the consumer advisory board and peer educators as resources for other community linkages.
  5. Distribute the list of resources to patients and families at planned visits. Have copies available in the waiting and exam rooms.

Tips
  • Check first to see if another community agency, such as AIDS Service Organizations (ASOs), National Association of People With AIDS (NAPWA), or National Minority AIDS Chapter (NMAC), has already developed a resource guide.
  • Create a notebook or computer database of resources.
  • Update information routinely.
  • Assign the role of keeping updated on community activities to a team member or staff person willing to become involved.
