This tool is used to document each response to a Rapid Response Team call. The form may be completed by the Rapid Response Team or the bedside nurse responsible for the patient. The embedded SBAR (Situation, Background, Assessment, Recommendation) section is helpful for framing the conversation with the team or the provider (physician).
