The purpose of the Rapid Response Team is to support hospital personnel outside of the emergency department and adult critical care units with early intervention in adult patients who demonstrate acute changes and/or are progressively deteriorating. This protocol is to be used in conjunction with Rapid Response Team Standardized Procedures.
This tool contains a Rapid Response Team Protocol as well as a Rapid Response Team Event Record.
- Rapid Response Team Protocol: Describes team member roles and responsibilities, patient assessment criteria, and guidelines for activating the team.
- Rapid Response Team Event Record: Allows the team to effectively and quickly document key information during an event. It has the added advantage of providing key information for data extraction:
