This tool incorporates three tools into one half-page, double-sided form. Included in the tool are the following:
- An SBAR communication tool, a framework for communication between members of the health care team about the patient's condition during a critical situation
- A checklist for reviewing clinical indices (or triggers) that are helpful in determining when to call the Rapid Response Team
- A Rapid Reponse Team data collection form
