Improving Chronic Illness Care (ICIC), a national program of The Robert Wood Johnson Foundation, is dedicated to the idea that United States health care can do better. The 99 million Americans who suffer from diabetes, depression and other chronic conditions can lead healthier lives. Providers who care for chronically ill patients can be better supported with guidelines, specialty expertise and information systems. Overall health care costs can be lowered through better care delivery.

Leadership
The Health Resources and Services Administration (HRSA) Bureau of Primary Health Care (BPHC), in partnership with the Institute for Healthcare Improvement, has embarked on a nationwide initiative to improve care for people with chronic conditions. The Health Disparities Collaboratives strive to achieve excellence through goals such as building strategic partnerships, transforming clinical practice through models of care, improvement, and learning, developing infrastructure, and generating/documenting improved health outcomes. The website includes training manuals, tools and resources, and upcoming opportunities to get involved.

Patient, Family & Consumer Sites
This comprehensive website includes information on diabetes as well as self-management. Presently there are active Message Boards to share ideas and opinions on a variety of topics with people affected by diabetes. In addition, the Diabetes Learning Center promotes better diabetes self-management in the sections entitled Nutrition, Exercise, and Weight Loss. The Virtual Grocery Store focuses on healthy food choices and meal planning. Much of the information is offered in Spanish.
