Percentage of Patients Who Had a Documented Foot Exam in the Last 12 Months


Definition

The number of diabetic patients in the registry who had a documented foot exam in the last 12 months, divided by the total number of diabetic patients in the registry. Multiply by 100 to get the percentage.


Goal

Greater than 90 percent of patients will have a documented foot exam in the last 12 months.


Data Collection Plan

On the last workday of the month, search the registry for all patients with a diagnosis of diabetes mellitus who had a documented foot exam in the last 12 months. Also, count the total number of patients with a diagnosis of diabetes mellitus.
