Definition
Average Hemoglobin A1c (HbA1c) value for patients in the diabetes registry.
Goal
Less than 7.0 average Hemoglobin A1c value.
Data Collection Plan
On the last workday of each month, search the registry for all patients with a diagnosis of diabetes mellitus who have had an HbA1c in the last 12 months. Add all of these patients’ most recent HbA1c values together and then divide by the number of such patients.
