Community:
Provide a List of Community Resources to Patients, Families, and Staff
    1. Identify existing lists of resources.
    2. List the shopping malls that are open early (and their hours) to accomodate walkers.  Find out whether there is a shuttle or mall security available to safely escort walkers to the bus stop.
    3. Assign a staff member or volunteer to collate lists, update information, and review for accuracy.  Collect contact information and guidelines for program use.
    4. Contact local Senior Information and Assistance Program (US federally mandated program) to avoid duplication of effort while building the resource guide.

Tips
  • Check first to see if another community agency has already developed a resource guide, such as the Chamber of Commerce list of organizations.
  • Create a notebook or computer database of resources.
  • Update information routinely.
  • Look in the phone book for Senior Information and Assistance (if in USA) — every community has this resource.
