Definition
The number of patients with a severity assessment at last contact (visit or phone) divided by the number of patients in the population of focus. Multiply by 100 to get percent.
Goal
95 percent of patients receiving a severity assessment.
Data Collection Plan
On the last workday of each month, from flow sheet/chart information or registry:
- Count the number of patients with severity assessment at last contact
- Count the total number of patients in the population of focus
Exclude patients who have not been entered into the registry.
