Aim
  • Increase the number of patients with persistent asthma who have a completed written Asthma Action Plan by 90 percent. 
  • Increase the number of patients with persistent asthma who are being treated with maintenance anti-inflammatory medication by 90 percent.
  • Decrease emergency room visits for the pilot population by 50 percent.


Measures
  • Percent of patients with persistent asthma who are being treated with maintenance anti-inflammatory medication
  • Percent of patients with persistent asthma who have a completed Asthma Action Plan
  • Percent of asthma patients with emergency room visits


Changes

The following changes were tested:

  • Identified the asthma population by chart audit and diagnosis
  • Scheduled visits for each asthma patient within the population
  • Completed an Action Plan for each asthma patient within the population
  • Educated each asthma patient within the population on what asthma is, proper use of inhalers, and how to use their Action Plan

  • Identified Asthma patients by providers to eliminate patients with Reactive Airway Disease or other respiratory illnesses.
  • Education of patients is a major key to compliance.
