Aim
- Increase the number of patients with persistent asthma who have a completed written Asthma Action Plan by 90 percent.
- Increase the number of patients with persistent asthma who are being treated with maintenance anti-inflammatory medication by 90 percent.
- Decrease emergency room visits for the pilot population by 50 percent.
Measures
- Percent of patients with persistent asthma who are being treated with maintenance anti-inflammatory medication
- Percent of patients with persistent asthma who have a completed Asthma Action Plan
- Percent of asthma patients with emergency room visits
Changes
The following changes were tested:
- Identified the asthma population by chart audit and diagnosis
- Scheduled visits for each asthma patient within the population
- Completed an Action Plan for each asthma patient within the population
- Educated each asthma patient within the population on what asthma is, proper use of inhalers, and how to use their Action Plan

- Identified Asthma patients by providers to eliminate patients with Reactive Airway Disease or other respiratory illnesses.
- Education of patients is a major key to compliance.
- Followed up on asthma at every scheduled visit.
